This helps you and the home health agency know earlier in the process if Medicare is likely to cover the services.

Agency Providers
Individuals with a High School Diploma and two years of experience providing service coordination to individuals with disabilities and/or seniors and knowledge about community resources. Home care is a health service provided in the patient's home to promote, maintain, or restore health or lessen the effects of illness and disability. Services may include nursing care; speech, physical and occupational therapies; home health aide services and personal care services. MDHHS is responsible for approving Home Help providers for participation in the program. The Expanded In-Home Services for the Elderly Program - provides non-medical services and supports for seniors who need help with basic daily activities, or require assistance with shopping, cooking, or paying bills.
An individual provider is defined by MDHHS as the direct care worker. The individual provider provides personal care services to an MDHHS Home Help beneficiary. Yes the applicant shall complete the initial application and select all of the licenses the agency is applying for on page 2 of the application and shall pay the higher of the licensure fees applicable. A home nursing agency or a home service agency shall pay a licensure fee not to exceed $1500. A home nursing placement agency or home services placement agency shall pay a licensure fee not to exceed $500. For a single home health agency license only, each initial and renewal application shall be accompanied by a license fee of $25.

The Department shall issue a new license to a new owner who meets the requirements for licensure under this Part. The transactions described in this Section shall not be complete until the Department issues a new license to the new person, legal entity, or partnership. The former licensee shall return its license to the Department by certified mail. In the case of agencies operating under a franchise arrangement, each unique business entity shall obtain and maintain a distinct license and shall not share licensure based on franchised name status. The agency must seek approval from the Department prior to caring for clients in a new county.
Formally review and submit a revised service plan as the applicable Waiver directs. Provide direct care management services, maintaining accurate and verifiable documentation of all services rendered. Services may be provided through an agency or be directed by the consumer, if allowable under the program.

The home health agency should give you a notice called the Advance Beneficiary Notice" before giving you services and supplies that Medicare doesn't cover. The health care provider shall complete the IDPH “Facility Information Change Form” prior to the relocation and submit to the Department. Whenever ownership of an agency is sold from the person or organization named on the license to another person or organization, the new owner shall apply for a new license.
